SEGFAULT world progression is now mapped as 4 worlds x 4 episodes with modular modifiers.

SEGFAULT progression is now formalized into a 4x4 world-episode matrix. Each world introduces modifier families that change traversal and threat timing.

Current effort is making those modifiers interoperable with the core movement contract, so difficulty comes from systems interaction rather than input friction.

Boss gating and escalation pacing are now tracked as explicit design constraints.
